Creating Your Account

Step 1: Log On 

• After being certified by your Certifying Health Care Provider, log on to the Medical Use of Marijuana Program Online System (Online System) and select I am a newly certified patient and I would like to create an account and register in the Medical Use of Marijuana Online System for the first time.  

Step 2: Enter Your Patient Identifying Information

• Enter your identifying information exactly as it is listed on the instructions provided to you by your Certifying Health Care Provider.
• You will need your PIN provided to you by your physician.

Step 3: Read Instructions

• On the next page, read the registration instructions, then click “Proceed” to continue the application. Enter your personal information in the provided field.

Step 4: Read Attestation

• Read the attestation and select the checkbox to agree. Click “Proceed.”

Step 5: Submit ID

• If you selected a Massachusetts Driver’s license or state ID as your form of identification and the system is unable to retrieve your photograph from the Massachusetts Registry of Motor Vehicles (RMV), or you are using a government ID not issued in Massachusetts, you will be redirected to the Identification Verification page. (If you are using a form of identification that is not issued by Massachusetts, click “Proceed.”)
• Review your information and make corrections, as needed.
• Click “Proceed” to continue.

Step 6: Submit Proof of Residency 

• On the following page, you will select the file containing your scanned proof of residency document and upload it by clicking the “Upload” button. You will then see the document listed in a table above the form. 
• Click “Proceed” to continue.

Step 7: Upload Identification Photo

• If the system was able to obtain your photograph from the Massachusetts RMV, you will see the photo displayed. You will have the option to use this photo or select a passport style photo of your own.
• Photos must be:
• In color;
• A square photo in portrait/upright format;
• Taken in front of a plain white or off-white background;
• Taken within the last 6 months;
• Showing only your head and the top of your shoulders;
• Taken looking directly at the camera held at eye level;
• Taken with both eyes open, and without eyewear; and
• Taken without any item that covers your face or head, except for religious purposes.
• Click “Proceed” to continue.

Step 8: Review Information

• Review your application information, then click “Submit.” If you need to update any section, click on the information to return to the page.

Step 9: Print your temporary Program Registration Card

• Once registration is approved, you will have an extended temporary Program Registration Card under the home tab that can be printed and presented for purchase at a Medical Marijuana Treatment Center (MTC).

Selecting a Personal Caregiver 

You have the option of designating a Personal Caregiver to assist you with your medical use of marijuana. After your registration has been approved by the Program, you may log into the Online System and designate up to two Personal Caregivers.

To select a Personal Caregiver, please follow these steps: 

• Step 1: Log into the Online System. 
• Step 2:Click on “My Caregivers” at the top of the page.
• Step 3: Click “Generate PIN” to create a personal identification number (PIN) for your Personal Caregiver. 
• Step 4: Provide the PIN to your Personal Caregiver and direct them to register with the Program in the Online System.  
• Step 5: After your personal caregiver has registered, click on the link “My Caregivers” at the top of the page and follow the instructions to approve your personal caregiver.
• Step 6: After you approve your Caregiver, the Program will process your Personal Caregiver’s registration.
